The coordinator will assess and evaluate individual needs, developing strength-based, stage-wise treatment plans matched to person-centered goals while maintaining consistent caseload contact. Responsibilities also include engaging individuals empathetically, screening for co-occurring disorders, and completing necessary documentation for community-based services and collaboration with partners.
Candidates must be a CMHP according to Medicaid guidelines, requiring either a Bachelor's degree with three years of supervised experience treating minors and families, or a Master’s prepared LLMSW or LMSW in Michigan with specialized training and one year of relevant experience. A valid motor vehicle operator's or chauffeur's license is also mandatory.
