As a Behavioral Health Assessment & Care Coordinator, you will serve as the first clinical connection for individuals and families seeking support, helping them access the services they need to thrive. This role is ideal for a licensed clinician who enjoys assessment, clinical decision-making, and collaborating with a multidisciplinary team to create meaningful pathways to recovery. You'll play a vital role in ensuring that individuals receive timely access to high-quality mental health services while making a lasting impact in your community.

Conduct comprehensive behavioral health assessments, diagnostic evaluations, and risk screenings.

Determine eligibility and appropriate levels of care based on clinical presentation and individual needs.

Collaborate with clients to identify strengths, goals, and service preferences.

Develop recommendations and facilitate connections to mental health, substance use, crisis, and community-based services.

Partner with psychiatrists, nurses, case managers, and other providers to coordinate care.

Maintain timely and accurate clinical documentation in the electronic health record.

Provide support and education to individuals and families throughout the admission and engagement process.

Participate in interdisciplinary treatment planning and quality improvement initiatives.

Ensure compliance with all regulatory, ethical, and professional standards.

Offer after hours crisis and daytime consultation and support for QMHP staff.

Serve on the Critical Incident & Stress Management (CISM) team to offer support services to disaster victims when needed.

This position is directly supervised by the Mental Health Program Administrator for Admissions.

Qualifications: Required -

Current Texas licensure as an LPC, LCSW, or LMFT, Master's degree in Counseling, Social Work, Psychology, Marriage and Family Therapy, or a related behavioral health field.

Strong clinical assessment, diagnostic, and treatment planning skills.

Excellent communication and documentation abilities.

Commitment to recovery-oriented, trauma-informed, and person-centered care.

Must be eligible to be certified as a Qualified Mental Health Professional.

Must possess a valid Texas Drivers License and be an insurable driver. Driving is an essential function and employees must maintain an acceptable driving record/insurability while employed.

Preferred -

Experience in community mental health, crisis services, outpatient treatment, or integrated behavioral health.

Knowledge of co-occurring mental health and substance use disorders.

Experience utilizing electronic health records and clinical documentation systems.

Training in trauma-informed care, suicide prevention, crisis intervention, or evidence-based practices.

This is a full-time position with standard work hours of Monday through Friday, 8:00am – 5:00pm. Will consider virtual/hybrid schedule.

Requirements

Candidates must possess a Master's Degree in Counseling, Social Work, or Psychology and hold a current license as an LPC, LCSW, LMFT, or Psychologist. Previous mental health work experience with adult consumers and a valid, insurable Texas Driver's License are also mandatory requirements.
