Minimum Qualifications: Required Education and Experience
Bachelor’s degree, preferably in Social Work, Psychology, or some related field
Associate's degree with an additional 2 or more years' experience working with educating and teaching clients about self-care, improving daily living activities
Total combined years of schooling and experience (working with educating and teaching clients about self-care, improving daily living activities) to equal 4 or more years
Required License/Certifications/Registrations
CPST Case Management training, KHS training, pass KBI, DCF child abuse check, adult abuse registry, and motor vehicle screens.
Completion of Adult Case Management training within 60 days of employment.
Valid driver’s license, insurance, and a dependable vehicle if position requires driving.
Occasionally exerting up to 25 lbs - frequently exerting up to 10 lbs. 11-25% of the day may be standing or walking.
