The Patient Navigator assists patients and families by coordinating appropriate referrals and resources based on physical, mental, and social assessments, acting as a liaison between acute and sub-acute providers for post-discharge care needs. Responsibilities also include coordinating evidence-based practices, providing education, supporting readmission prevention efforts, and developing data dashboards to enhance patient population coordination.
Candidates must possess a Bachelor's degree in nursing and have at least 3 years of experience in an acute clinical setting relevant to their expertise. Required credentials include a current and unrestricted Louisiana RN license and BLS certification, along with proficient English verbal, written communication, and computer skills.
