Schedule

• What you will do: Promote/provide physical therapy and/or occupational therapy services in the home to assist the patient in achieving his/her optimal level of functioning. Evaluate and treat individuals with functional deficits secondary to neurologic, orthopedic and medical conditions. Teach patients/caregivers and help them to become independent with their treatments and self-management of their chronic illness(es) Experience the rewards of appreciation from being able to address the whole patient Coordinate an interdisciplinary plan of care with nursing/physical/occupational/speech therapists, medical social workers, aides and dieticians.

• What you will need: Current NYS PT or OT license and registration Minimum of one (1) year of experience as a PT or OT, preferably inpatient or homecare, is required Home care experience preferred, but not required Valid Driver's License and Reliable Vehicle. This position provides mileage reimbursement.
