The Advanced Practice Provider is responsible for the proficient management and coordination of patient care within their assigned population in post-acute care facilities, which includes coordinating and delivering exceptional patient care and communicating effectively with nursing staff and collaborating physicians.
Candidates must possess a required Nurse Practitioner License and DEA Certification, with one year of relevant experience preferred, ideally in Geriatrics, Internal Medicine, ICU, Emergency Medicine, Hospitalist, or General Practice.
