Nurse Practitioner - Post Acute Cumberland County

Major function

• To provide palliative services in the hospital setting at IMC- Mullica Hill.
• The practitioner will be assigned a specific patient load for which they will be responsible.
• This role will collaborate with our Palliative physicians to assess patients and provide palliative care to best treat our patients.
• APN will be responsible for developing and providing services for the Palliative Care program.
• In addition, will work collaboratively with physicians, nurses, case managers, clergy, hospice, and home care to meet the needs of patients with chronic progressive disease or terminal disease.
• This role reports directly to the physician director for palliative care services at Inspira.

Qualifications

Education & Experience

• MSN required.
• Successful completion of advanced practitioner training and certification as a practitioner by an accredited nurse practitioner program and certifying body.
• Minimum 3 years professional nursing (Nurse Practitioner or Clinical Nurse Specialist) in adult health and chronic disease management, pain management and palliative care.
• Critical Care experience preferred.
• Experience providing APN care for geriatric clients preferred.

Certification/Licensure

• Current APN and DEA license in NJ.
• RN license in NJ, ACLS preferred, and National Certification in area of specialty.
• Current advance practice nurse certification preferred
