The Care Management Clinician provides case management, crisis intervention, and clinical care coordination for children and their families. They are responsible for conducting mental health assessments, developing individualized care plans, and collaborating with interdisciplinary teams to ensure continuity of care.

Conducts assessments of patients' mental health needs and develops individualized care plans. Monitors patients' progress and adjusts care plans as needed.
Coordinates client care with other service providers and collaborates with interdisciplinary team to ensure continuity of care.
Coordinates available NCH and community resources to enhance client care.
Provides education to patients, families, and care teams about mental health conditions and treatment options.
Performs brief therapeutic interventions and counseling.
Completes clinical charting and other related administrative paperwork in accordance with policy standards.
Participates in quality improvement initiatives to improve patient outcomes.
Master's degree in Counseling, Social Work or Marriage and Family Therapy, required.
