The Social Worker supports the hospice interdisciplinary team by completing comprehensive patient assessments, identifying care planning goals, and providing psychosocial and emotional support to patients and caregivers at the end of life. Responsibilities also include documenting assessments in the EMR, participating in team meetings, and conducting home visits.
Candidates must possess a Bachelor of Social Work (BSW) and preferably a Master of Social Work (MSW), with specific licensure requirements varying by state (Tennessee, Missouri, Virginia, South Carolina, Georgia). A minimum of one year of social work experience in a healthcare setting and excellent communication skills are required.
