The Nurse Practitioner will address suffering in aging populations with dementia and neurocognitive disorders by traveling daily to assigned facilities to provide initial evaluations, continuing therapy services, and cognitive testing. Responsibilities also include documenting service needs and maintaining positive professional relationships with facilities, patients, and families.
Candidates must possess proof of Nurse Practitioner licensure and the ability to function autonomously while collaborating within an interdisciplinary treatment team, supported by good time management skills. Successful candidates must also pass a pre-employment background check, drug screening, and complete the credentialing process.
As a Nurse Practitioner at GuideStar Eldercare, you will have the opportunity to address the suffering of an aging population afflicted with dementia, Alzheimer’s disease, and other neurocognitive disorders in nursing homes and assisted living facilities. Experience the autonomy of private practice, but with an assigned caseload, the collaboration of corporate clinical leadership, and the administrative support of our in-house scheduling, coding, and billing departments.
