The Resident Care Coordinator is responsible for coordinating client care, including scheduling and communication for appointments, transportation, and insurance matters. This role also involves guiding individuals toward independent community living and participating in developing individual program plans and coordinating specialized services.
Candidates must meet specific education and experience thresholds, such as an Associate's degree with 4 years of experience, a Bachelor's degree with 2 years of experience, or a Master's degree with 1 year of experience, all requiring direct work experience with individuals with intellectual disabilities or autism. Pre-employment screening includes criminal background checks, registry checks, drug testing, and education verification.
Mon - Fri 8am - 4pm, depending on Lab work and other scheduled doctor appointments.
A master’s degree or above from an accredited college or university and 1 year of work experience working directly with individuals with an intellectual disability or autism. A bachelor’s degree from an accredited college or university and 2 years of work experience working directly with individuals with an intellectual disability or autism. An associate’s degree or 60 credit hours from an accredited college or university and 4 years of work experience working directly with individuals with an intellectual disability or autism. Quickmar software Responsibilities: Coordinate and schedule medical, psychiatric and dental/vision appointments with doctors' offices, health care providers, laboratories along with communicating and liaison with physicians, nurses and other relevant behavioral health and health care professionals regarding the medical needs of the resident. ( 65%) Track all physical, health care ,dental , vision appointment . (10%) Prepare and complete all necessary documentation in accordance with program standards within required time frames as required. ( ie. MA 51) (10%) Perform Direct Support Services when needed (10%). Work on computer to perform the above duties - ongoing These are not all encompassed job responsibilities. Other duties may be assigned as necessary due to the needs of the program and residents served. Pre-employment screening: Complete criminal background Name checked in the registries. (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities) Drug testing Education verification and other credentialing based on position requirements.
