The Physical Therapist in Home Health works collaboratively with an interdisciplinary team to deliver high-quality physical therapy services in patients' homes. Responsibilities include evaluating patients, establishing care plans, providing treatment, and documenting patient progress.
Candidates must have a Doctoral, Master's, or Bachelor's degree from an accredited physical therapy program and be licensed by the Pennsylvania State Board of Physical Therapy. One to two years of home health experience is preferred, along with current CPR certification.
