The Registered Nurse will collaborate with physicians and preceptors to implement, evaluate, and follow up on patient care plans based on ongoing assessments of physical, emotional, social, educational, and functional needs. Responsibilities also include interpreting assessment data to develop comprehensive nursing care plans and actively participating in the interdisciplinary team for rehabilitation goal planning and evaluation.
In collaboration with the physician and your preceptor, you will ensure implementation of the assessment plan, evaluation, and follow-up of the plan of care for the patient population. Initiating an ongoing systematic assessment of the physical, emotional, social, educational and functional needs of the patients/families. Interpreting assessment data and information to develop a nursing care plan which contains problem identification, nursing diagnosis, nursing orders, approaches, interventions, short/long term goals and discharge planning. Participating as a productive member of the interdisciplinary team in assessment, planning, implementation and evaluation of rehabilitation goals. Assuring that documents in the medical record are complete, factual, accurate and timely.
Applicants must be graduates of an accredited registered nursing program and must possess a valid State RN License by the start date, although they may apply before licensure. A minimum requirement is holding a Basic Life Support (BLS) certification at hire, along with flexibility in scheduling to meet program requirements.
