RN Case Manager

This role involves providing basic and skilled nursing care, setting priorities of care, and appropriately assessing intervention limits based on clinical expertise. The RN Case Manager must communicate effectively with patients, families, and team members regarding care issues while working both independently and within an interdisciplinary team.

Candidates must be a licensed Registered Nurse in Ohio, with the ability to obtain a Michigan license, and preferably possess a BSN and at least two years of medical-surgical nursing experience, with homecare or hospice experience being a plus. Essential requirements include proficiency in EMR charting, strong clinical skills, excellent communication, and maintaining current CPR certification.
