This PRN Nurse Practitioner provides as-needed clinical support, including patient visits, symptom management, and patient/family education across various settings. Responsibilities also involve timely documentation and close collaboration with the hospice team and Medical Director.
Candidates must possess a current Wisconsin Nurse Practitioner license and an active DEA or willingness to obtain one. Hospice or palliative care experience is preferred, alongside strong communication skills and the ability to work independently.
Key Responsibilities Provide PRN visits for hospice patients in homes, facilities, or community settings Support symptom management, medication adjustments, and patient/family education Complete face-to-face (F2F) encounters as assigned Document timely and accurately in the EMR Collaborate with the hospice team (RN case managers, social workers, chaplains, aides) Communicate changes in condition to the Medical Director and DCS Participate in IDG meetings as needed
Current WI Nurse Practitioner license (APRN) Active DEA or willingness to obtain Hospice or palliative care experience preferred (not required) Strong communication, critical thinking, and compassionate bedside presence Ability to work independently and provide timely visit availability Why Join Allegiant Hospice?
