The Surgical Care Coordinator manages the continuum of care for surgical patients, ensuring outstanding customer service and fostering an environment of teamwork. Responsibilities include assisting in patient identification for navigation, assessing barriers, providing education, and coordinating appointments and follow-up care.
Candidates must be licensed as a Practical Nurse in Pennsylvania with a minimum of three years of related experience, including strong clinical skills, particularly in orthopedics and chronic care management. Proficiency in clinical paperwork and electronic medical records, along with high analytical and critical thinking skills, is mandatory.

The Surgical Care Coordinator is responsible for the coordination of patient care and management of surgical patients through the continuum of care. The Surgical Care Coordinator will provide outstanding customer service and ensure staff members provide the same. The incumbent will foster an environment of teamwork and serve as a role model in the department. They will demonstrate actions and attitudes that contribute to the critical success of patient navigation in a community and inpatient setting.
Support Saturday surgeries and vacation coverage.
