The Case Manager will deliver community-based services to eligible individuals needing behavioral health support, focusing on assessing and monitoring resources to ensure clients live safely and stably within the community. Key duties include managing a caseload of up to 30 clients, developing individualized, strength-based service plans, and assisting consumers in obtaining and maintaining basic needs like housing and healthcare.
Candidates must meet specific education and experience criteria, which includes either a bachelor's degree in a relevant field or being a registered nurse, or possessing a high school diploma combined with specific social science coursework and relevant human services experience. Required clearances include Pennsylvania Child Abuse, Criminal/Federal History checks, a valid driver's license, and verification of no Medicaid/Medicare exclusion.

Full-Time, Monday through Friday; 9:00 am - 5:00 pm

Pay Rate: $20+/hour
Pennsylvania Child Abuse Clearance, Criminal and fingerprint-based federal criminal history; Valid driver’s license; Verification that employee is not on any Medicaid/Medicare Exclusion list.
At the discretion of the Personnel Officer, additional related experience and/or education may be substituted instead of the requirements specified under Education and Experience
