The Outreach Nurse delivers clinical care, preventive screenings, and health education in community settings to improve access to care for underserved populations, integrating nursing practice with population health goals. This role focuses on closing care gaps, managing chronic diseases, and converting outreach encounters into scheduled and completed primary care visits.
include a current and valid Licensed Practical Nurse (LPN) license in Missouri and completion of an accredited practical nursing program, along with Basic Life Support (BLS) certification. Candidates should possess strong clinical assessment skills, knowledge of population health strategies, and proficiency in EHR systems.
The Outreach Nurse – Mobile Health & Community Engagement is responsible for delivering clinical care, preventive screenings, and health education in community-based settings to improve access to care for underserved populations. This role integrates nursing practice, population health, and patient navigation to support care gap closure, chronic disease management, and preventive health initiatives.
The Outreach Nurse ensures that all outreach services and education align with organizational capabilities and are based on identified community health needs in accordance with AAAHC and HRSA standards. This position plays a key role in connecting community members to primary care services and converting outreach encounters into scheduled and completed clinical visits.
