The Licensed Practical Nurse (LPN) provides direct patient care, including specialized end-of-life care, following the established plan of care and adhering to the State Nurse Practice Act. Responsibilities also include accurate documentation, patient/family education, coordinating hospice services, and managing pain and symptoms.
Candidates must be a graduate of an accredited practical nurse program with three years of nursing experience, preferably in community health, hospice, or medical/surgical settings. Applicants must hold a current LPN license in Vermont and possess good communication and organizational skills, along with the physical ability to perform patient transfers and lift equipment.
The Home Health and Hospice Licensed Practical Nurse (LPN) is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.
This position has no supervisory responsibilities.
