The role involves conducting comprehensive patient assessments and managing members through acute medical situations or exacerbations of chronic conditions, including ordering testing, diagnostics, and interventions. Responsibilities also include managing members during short-term sub-acute rehabilitation, ensuring accurate documentation, collaborating with the care team, and conducting advanced illness and care planning conversations.
Applicants must be graduates of an accredited Master of Science Nursing or Doctor of Nursing Practice program for Nurse Practitioners, or an accredited Physician Assistant degree program for Physician Assistants, with active, unrestricted licenses in Oregon or the ability to obtain one. Certification by the appropriate national board (AANP, ANCC, or NCCPA) and current DEA licensure/prescriptive authority are also required.
1+ years of clinical experience in practice (long-term care setting preferred) Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs Understanding of Geriatrics and Chronic Illness Understanding of Advanced Illness and end of life discussions Proficient computer skills including the ability to document medical information with written and electronic medical records Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
