The Intake Assessor is responsible for facilitating the response to service requests, including initial response, assessment, referral, determining the appropriate level of care, and managing admissions and follow-up activities. This role involves conducting initial evaluations, documenting patient symptoms and needs, and collaborating with the on-call physician for level of care determination.
Candidates must possess an RN Degree and an active RN license, along with a valid driver's license and proof of insurance if using a personal vehicle. Prior experience in a psychiatric healthcare facility working with adult and geriatric patients is preferred, and candidates must obtain CPR and Handle with Care certifications within 30 days.
