The Staff Nurse provides skilled nursing care to patients in their homes, following physician orders and care plans. They are responsible for evaluating patient needs, coordinating services, and documenting care provided.
Candidates must be graduates of an accredited nursing school with an associate degree and have at least two years of nursing experience. Current RN licensure in Ohio, CPR certification, and reliable transportation are also required.
Weekend position - 12 hr shifts Saturday and Sunday with additional 8hrs paid weekly to equal 32hrs/week.
The Staff Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Clinical Manager/Administrator, and in accordance with departmental and agency guidelines and policies.
Graduate of an accredited school of nursing.
Associate degree
Two years of professional nursing experience in hospital or community health nursing prefered.
Current RN licensure by the State Board of Nursing in the State of Ohio.
CPR certification/BLS.
Current State of Ohio drivers license
Reliable transportation and auto liability insurance.

Ability to care for patients of all ages to include neonates, children, adults, adolescent and geriatric population.
Ability to communicate effectively with patients and families, other members of the home health care team, the medical staff and the community.
Ability to work independently in providing patient care and carrying out duties
Provides services in accordance with the plan of care.
Makes the initial evaluation visit and regularly reevaluates the patient's nursing needs.
Initiates the plan of care and necessary revisions.
Provides those services requiring substantial specialized nursing skills.
Initiates appropriate preventative and rehabilitative nursing procedures.
Prepares clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy.
Coordinates services.
Informs personnel of changes in the condition and needs of the patient.
Counsels the patient and family/significant others in meeting nursing and related needs.
Participates in and presents inservice programs.
Understands and adheres to established Agency policies and procedures.
Processes orders and notifies physician of patient needs and changes in condition. Completes certification/recertification orders and discharge summaries.
Determines the amount and type of nursing needed by each individual patient.
Refers to Physical Therapist, Speech Language Pathologist, Occupational Therapist and Medical Social Worker those patients requiring their specialized skills.
Conducts patient care conferences on patients assigned to his/her care.
Participates in peer review and Quality Assessment and Performance Improvement as assigned.
Gives total patient care as needed.
Takes on-call duty nights, weekends, and holidays, as assigned.
Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames.
Appropriately utilizes ICD-10 codes.
Positively represents the department in interdepartmental interactions.

Attends all mandatory education programs and can describe his or her responsibilities related to general safety and regulatory compliance.
Appearance is professional, neat, clean, and appropriate for the work environment. Wears proper photo identification badge while on duty.
Maintains confidentiality of all hospital and patient information at all times, as observed by peers and management.
Attends monthly staff meetings and participates in monthly rounding process with manager to assure effective and timely two-way communications.
Participates in and completes required department specific education and training programs and can describe his or her responsibilities related to department safety and specific job-related hazards.
Follows safety and security policies including exposure control plans/blood borne and airborne pathogens.
