The Clinical Therapist will provide neurobehavioral health services, including evaluation and condition-specific or insight-based therapy, to residents in long-term care facilities. Responsibilities also include coordinating the patient’s care plan with the facility, PCP, colleagues, and engaging with the family as needed.
Candidates must possess a Master’s degree in Counseling or Social Work and hold current licensure as an LPCC or LISW in the state of application. Required attributes include the ability to function effectively within an interdisciplinary treatment team, good time management, and the capacity to work autonomously with managerial support.

As a Licensed Professional Clinical Counselor (LPCC) or Licensed Independent Social Worker (LISW) with GuideStar Eldercare you will provide neurobehavioral health services to residents in long-term care facilities. Using a multi-disciplinary approach, you will partner with specialty clinicians to provide holistic, continuity of care for elderly individuals who are afflicted with neurocognitive disorders and other neurologic and/or psychiatric conditions.
