The Registered Nurse will provide and coordinate multidisciplinary health services for patients in the community, working closely with physicians and other team members. Responsibilities include managing patient care plans, supervising health workers, and ensuring compliance with therapy and documentation requirements.
Candidates must have a valid RN license in New York State and preferably a BSN. A minimum of one year of experience in a hospital or CHHA setting is preferred.
FlexStaff is seeking a Registered Nurse with CHHA or hospital experience for our client, a CHHA agency. This is a full-time, Monday-Friday, either 9a-5pm or 8a-4pm, depending on needs.
License and current registration to practice as a Registered Professional Nurse in New York State. Graduate of an accredited school of nursing, BSN preferred. A minimum of one year experience in hospital and or minimum of one year experience in a CHHA preferred. You will be responsible for providing and coordinating multidisciplinary health services for patients in the community. Works with the physician and other team members in implementation of plan of care.
Coordinates cost-effective patient care that meets insurance and physician approval. Verifies insurance plan details and checks patient ID cards at home. Accepts case assignments based on client needs, regardless of location. Handles all CHHA requirements, including assessments and daily documentation. Travels to patients' homes/facilities to provide direct care. Admits patients promptly as needed. Completes Oasis in-person, accurately reflecting patients' clinical status, including diagnosis coding. Collaborates with the Director and physician to determine primary diagnoses. Participates in meetings to report service adherence and patient progress for discharge planning. Communicates with PT/OT to ensure compliance with physician-ordered therapy. Serves as the coordinator for the multidisciplinary care team. Develops care plans with input from health team members, focusing on patient and family goals. Documents all services provided. Monitors care plan effects and updates the team on any changes in patient needs. Adjusts care goals and interventions as necessary. Educates patients and families on disease management, self-care, and prevention. Initiates and maintains communication for optimal patient care. Supervises health and personal care workers via home visits. Reviews and updates the HHA plan of care with workers and families. Communicates regularly with HHAs regarding care plans and patient goals. Documents initial and bi-weekly HHA service supervision. Plans and documents patient discharges from the program. Manages an assigned caseload in Manhattan.
