About the role

The Licensed Mental Health Professional is responsible for providing assessment and service planning, quality assurance, and coordination of care for individuals with mental health needs. They must also maintain case records and provide culturally sensitive treatment to clients and their families.

Job definition

Work with the CN Guidance Critical Time Intervention Team to deliver services within an evidence-based model that is time limited, phase-based care management approach focused on enhancing continuity of care during transitional times. The Mental Health Professional resolves clinical and rehabilitative needs to meet the individual's recovery goals.

**You will receive a sign-on bonus of $5,000, provided you remain employed in this role in a fulltime capacity for 6 full months.**

Educational requirements

Licensed Psychologists, Licensed Clinical/Masters Social Workers, Licensed Marriage and Family Therapists, Licensed Mental Health Counselors, or Licensed Creative Arts Therapist. Limited permit permissible.

Experience required

Two years' experience working with the chronically mentally ill (Masters level).

Duties: And responsibilities:

  • 1.
  • Responsible for providing Assessment and Service Planning, Quality Assurance and Improvement Processes, Utilization Review, incident reporting and coordination between CTI and other systems.
  • 2.
  • Provide culturally sensitive and developmentally appropriate treatment to individuals and their families.
  • Services include but are not limited to Assertive Engagement, Case Management, Assessment, Family Counseling/therapy, Family Psychoeducation, Individual Psychoeducation, Psychosocial Rehabilitation, Crisis Intervention Services 3.
  • Maintains case records and completing documentation on time and in a matter in compliance with federal, state and agency regulations.
  • 4.
  • Provides direct, face-to-face and/or collateral services to recipients either in Office or in the community.
  • 5.
  • Must be available on an as needed basis 24 hours a day, 7 days a week, including responsibility for possible afterhours coverage on the phone and in person as needed.
  • 6.
  • Assists recipients in developing appropriate expectations from treatment.
  • 7.
  • Exchanges information with other agencies that benefits the recipient or ensures compliance with regulations and coordination of services.
  • 8.
  • Utilizes and understands the value of supervision, which increases practice skills, promotes better services for the recipient and preserves accountability to the Agency.
  • 9.
  • Participate in staff development and providing staff supervision and staff training.
  • 10.
  • Actively engage in agency wide trainings and work groups as well as NYS specific training on and for the CTI model.
  • 11.
  • Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress 12.
  • Complete QI Training during onboarding and participate in QI refresher training as needed.
  • 13.
  • Ability to cultivate a culture of inclusion for all employees that respects their individual strengths, views and experiences.
