The Registered Nurse is responsible for the assessment, planning, and implementation of nursing care for patients in a home care setting. They also supervise non-professional personnel and collaborate with a multi-disciplinary team to ensure high-quality patient outcomes.

Candidates must be a graduate of an accredited Registered Nurse program and hold a valid Virginia RN license. A Bachelor's degree in Nursing is preferred, along with BLS/CPR certification and a valid driver's license.
Newport News, Virginia Overview Works under the supervision of the Nurse manager and the Director and is responsible for the assessment, planning, implementing, and evaluating the total nursing care of assigned patients on an intermittent visiting basis.
The RN may work in a variety of settings (typically in a home care type setting) and roles within Riverside Health System.
The RN utilizes communication, critical thinking, problem solving, and decision making to create a care plan for each patient based on physiological, psychosocial and cultural diversity.
Responsible for supervising and delegating non-professional personnel working directly with him/her to ensure optimum patient care.
Must demonstrate competence in dealing with age related needs of specific patient populations served in their assigned area.
Works under the direction of leadership and a multi-disciplinary team (physical, speech, or occupational therapy; medical social services, and Home Health Aide services) to provide a comprehensive range of services or clinical care associated with and in accordance with standards of excellence established by Riverside Health system and facility goals and strategic plans.
What you will do Demonstrates assessment ability by applying standards of practice as it relates to the specialty or patient population served.
Completes required continuing education classes applicable to Private Duty Care as assigned in a timely manner and stays current in regards to clinical knowledge.
Participates in multi-disciplinary team meetings and discussions regarding quality and outcomes.
Demonstrates the ability to develop an initial and ongoing comprehensive assessment of the clients needs.
Contributes to the development, maintenance, or modifications to the individual service plans.
Identifies changes in condition with proper documentation according to nursing practice standards.
Accurately completes all required clinical documentation consistently per RHS standards which includes completing and "locking" patient notes and verbal orders.
Applies specific criteria for admission to private duty care to establish appropriate levels of care and the patient's eligibility.
Employee's individual productivity measures are satisfactory based on FTE/status and average number of assigned patients in case load.
