Our Company Rehab Without Walls Neuro Rehabilitation Overview Physical Therapy Assistant (PTA) – Flexible / PRN (W2) Rehab Without Walls® Neurorehabilitation | Walnut Creek, CA Bring Outpatient-Level Care Directly to the Community Rehab Without Walls® is seeking an experienced Physical Therapy Assistant (PTA) to join our innovative Home & Community Neurorehabilitation Program in Walnut Creek, CA.

This is a **W2 PRN position—not 1099—**offering stability, support, and flexibility while allowing you to deliver meaningful, real-world rehabilitation.

Our model brings outpatient-quality care directly to the client, supporting independence and reintegration where life actually happens.

Who We’re Looking For: An experienced PTA seeking a primary therapist assistant role from first treatment through discharge.

Interested in a caseload of approximately 15–20 hours per week across 3 days, with consistency and continuity of care.

Passionate about functional rehabilitation in real-life environments, such as homes and community settings.

Ready to move beyond staged therapy and support clients as they: Safely pick up and manage their own prescriptions Return to driving Transition back to work Participate in yoga, light hiking, and other meaningful activities A clinician who thrives independently while valuing collaboration with OTs, PTs, SLPs, and other specialized clinicians.

Responsibilities

Provides physical therapy services planned, delegated and supervised by the qualified Physical Therapist in accordance with the patient’s Plan of Treatment Confers with Physical Therapist regularly regarding patient’s plan of treatment, status of progress toward goals, questions and concerns Assists with cleaning and maintenance of equipment, treatment and department areas Reports any problems with department equipment to the appropriate personnel to ensure that it is maintained in good working order Communicates patient’s needs and progress to the Physical Therapist of record and assists in securing special adaptive equipment for patient to improve function. Incorporates the use of adaptive equipment to enhance the rehabilitation program Maintains and submits patient treatment and billing documentation in the format and timelines required by the Company, facility, funding source, local, state and federal regulations

Qualifications

Current unrestricted license as a Physical Therapist Assistant by state in which practicing Current Basic Life Support or cardiopulmonary resuscitation certification, in accordance state regulations A minimum of one year’s work experience as a Physical Therapist Assistant preferred Demonstrates knowledge of rehabilitation techniques related to complex neurological injury Communicates effectively and professionally in verbal and written interactions Demonstrates strong organization skills and attention to detail About our Line of Business Rehab Without Walls, an affiliate of BrightSpring Health Services, is a revolutionary neurorehabilitation program that moves individuals outside the walls of institutional settings and into their natural home and community environment through a continuum of care. Salary Range

USD $40.00 - $45.00 / Hour

Requirements

Requires a Master's Degree in Clinical Social Work and current LCSW licensure in the state of practice. Candidates must have at least one year of experience in a rehabilitation setting working with traumatic brain or spinal cord injuries.
