We’re seeking an experienced Physical Therapist for a flexible PRN, W2 role (not 1099) in Santa Cruz, CA.
This position offers consistency with a manageable caseload, organizational support, and the opportunity to make a powerful, real‑world impact.
Who We’re Looking For: An experienced Physical Therapist interested in serving as a primary therapist from evaluation through discharge.
Availability for approximately 15–20 hours per week, typically supporting 3 consistent clients.
Passionate about functional, real-world rehabilitation delivered beyond traditional clinic walls.
Motivated to help clients transition back to meaningful life activities, including: Independently picking up and managing prescriptions Returning to driving Transitioning back to work Participating in yoga, light hiking, and community mobility A clinician who thrives with professional autonomy while valuing collaboration with a multidisciplinary team of PTs, OTs, SLPs, and LCSWs.

What you will do: Responsibilities listed include but not limited to: Perform evaluations and develop treatment plans Regularly re-assess effectiveness of treatment plans, attend staffing meetings and family conferences Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources Communicate patient’s needs and progress to the treatment team, physician, person receiving services and family members Communicates with other disciplines to ensure collaboration, coordination of care and enhance patient outcomes
