The ICF Case Manager coordinates and oversees services for individuals in an Intermediate Care Facility, ensuring compliance with federal and state regulations. They develop Individualized Service Plans and collaborate with interdisciplinary teams to support the health, safety, and quality of life of residents.
Candidates must have a high school diploma or equivalent, with an associate or bachelor's degree in a related field preferred. A minimum of 2 years of experience working with individuals with developmental disabilities is required.
The ICF Case Manager is responsible for coordinating and overseeing services for individuals residing in an Intermediate Care Facility (ICF) setting. This role ensures compliance with all federal, state, and agency regulations while supporting person-centered planning, health, safety, and quality of life for individuals with developmental disabilities.
This is an on-site position at Parc Center for Disabilities, working as part of a collaborative, mission-driven team.
High School Diploma or equivalent required. Associate or Bachelor’s degree in Social Work, Human Services, Psychology, or related field preferred. Minimum of 2 years’ experience working with individuals with developmental disabilities required. ICF/DD or case management experience strongly preferred. Valid Florida Driver’s License. Required Skills & Competencies Knowledge of ICF/DD regulations and compliance standards. Strong organizational and documentation skills. Ability to manage multiple priorities in a fast-paced environment. Effective communication and teamwork skills. Proficiency in Microsoft Office and electronic health records. Commitment to person-centered care. Typical hours: Monday through Friday, 7:30 AM – 3:30 PM. This is an on-site position at Parc Center for Disabilities.
