The provider will deliver primary care services to residents in long-term care facilities, including performing physical exams and diagnosing acute or chronic conditions. They are also responsible for developing individualized care plans, prescribing medications, and collaborating with interdisciplinary teams to ensure high-quality patient outcomes.
Candidates must hold an active, unrestricted license as a Nurse Practitioner, Physician Assistant, or Physician in Massachusetts. A minimum of 6 months of experience in long-term care or a skilled nursing facility is required.
