The LPN is responsible for performing initial data collection on residents and implementing person-driven care plans developed by the RN. They utilize principles of relationship-based care to provide quality, patient-centered, and evidence-based nursing services.
Candidates must hold a current state licensure as a Practical Nurse in good standing. The role requires the ability to read, write, and communicate effectively in English, with clinical experience preferred.

The LPN is responsible for utilizing the principles of Relationship Based Care while performing duties. The LPN is responsible for initial data collection on all residents. The LPN participates in implementing person driven care plans as developed by RN.
Current State licensure as a Practical Nurse, in good standing. Must be able to read, write and communicate in English. Clinical experience preferred. $34.36 - $37.96 per hour. $2.25 – 2.50 shift differential for evening, night, and weekend shifts.
