The APRN/PA-C will partner with patients to understand their health history, perform comprehensive examinations, and develop evidence-based diagnoses. They will manage a wide range of acute and chronic conditions while ensuring optimal patient outcomes through safe medication management and patient education.
Concord Hospital Medical Group is seeking an experienced APRN or PA-C to join an established outpatient family medicine practice at Pembroke Family Physicians.
This is a four-day workweek role with no weekends, no call, and an average of 16 patients per day, designed for providers who want to deliver high-quality care without burnout. You’ll work in a collaborative, well-supported environment that values thoughtful medicine and strong patient relationships.

Candidates must have an unencumbered New Hampshire APRN or PA-C license and national certification in their relevant specialty. A Master's or Doctoral degree from an accredited program is required for APRNs, while PA-Cs must have a Master's degree from an ARC-PA accredited program.
