Field Nurse Practitioner / Physician Assistant (Acute Care in the Home)

• Perform problem-focused assessments and deliver therapeutic interventions (e.g., IV fluids, wound care, splinting, medication administration) in the home. No charge nurse, no transfer delays. You assess and act.

Who You Are • Licensed Professional

• You hold a current, unrestricted State license as a Nurse Practitioner or Physician Assistant.
• Experienced
• Clinician: You have at least 2 years (or 3,600 hours) of experience in relevant high-acuity settings such as the Emergency Department (ED), Inpatient Medicine, or Post-Acute Care.
• EMS
• Background (Highly
• Desirable): Experience in EMS, particularly with IV access and field medication administration, is a huge plus.
• Adaptable: You thrive in fast-paced startup environments and have the ability to adapt your clinical skills to the unique needs of patients in their homes in diverse home settings.
• Certified: Current BLS certification required; ACLS preferred.
• Mobile: You have a valid driver's license and the physical ability to carry equipment (up to 60 lbs) and navigate stairs.

• Flexibility is required for evenings, weekends, and holiday coverage to ensure our patients receive care when they need it most.
