The Care Coordinator provides therapeutic interventions, case coordination, and resource linkage to clients and their families. They are responsible for maintaining clinical documentation, participating in treatment team meetings, and advocating for clients across various agencies.
Candidates must hold a Bachelor's degree in a Human Service discipline and have at least one year of relevant experience in the mental health field. Strong knowledge of child development, behavior modification, and the ability to travel are essential for this role.

The Care Coordinator I provides Clarvida approved therapeutic interventions, case coordination, and resource linkage to clients and involved parties including families, DCS, court personnel, and community supports to support positive change within individuals and families. This role includes client assessments, treatment planning, crisis intervention, advocacy, and collaboration with multidisciplinary treatment teams to ensure high-quality client care and compliance with organizational and regulatory standards.
· Assess clients during intake as needed and conduct community client visitation · Transport clients and complete on-call duties as assigned · Provide Clarvida approved therapeutic services including coping skills, behavior modification, parenting skills, and crisis intervention · Work with treatment teams to create, personalize, and update Treatment Plans and required documentation · Ensure treatment plans align with Clarvida policies, state licensing regulations, and COA accreditation requirements · Serve as a liaison and advocate with professionals and agencies involved in client care · Attend Supervision and Treatment Team meetings and present assigned client updates as required · Support fellow team members to ensure the best outcomes for clients and families · Complete clinical documentation within established timeframes and correct deficiencies promptly · Complete peer audits as assigned and contribute to the Performance Quality Improvement Cycle · Collect and utilize data to continuously improve client and program services · Travel to multiple locations and maintain flexibility consistent with program needs Required Qualifications · Bachelor’s degree in a Human Service discipline from an accredited four-year college or university · 1 year of relevant experience in the Mental Health field · Sensitive to other cultures and socioeconomic levels · Thorough knowledge of child development and behavior modification · Strong organizational, communication, and mentoring skills · Ability to work flexible hours consistent with program needs · Ability to work collaboratively as a team member · Ability to travel and work at multiple locations
