The Case Manager assesses patient and family scenarios to establish effective plans for the continuum of care. They perform assessment, planning, intervention, and reassessment in a collaborative environment to optimize quality and utilization.
Candidates must hold a Florida Registered Nurse license and have a minimum of 5 years of experience. A Diploma or Associate degree in Nursing is required, while a Bachelor's degree and CCM certification are preferred.
The Case Manager utilizes professional nursing skills to assess patient and family scenarios for clinical appropriateness for the setting and establishes plans for effective management of the continuum of care. Assessment, planning, intervention and reassessment are performed in a collaborative environment. The key goals of the Case Manager include proactive, individualized planning for patient's progress that optimizes quality of care, utilization, cost and patient satisfaction.
