A Brief Overview

Evaluates patients, develops treatment plans and provides treatment for medically necessary speech therapy services addressing a patient's deficits in speech, language, cognition, voice and dysphagia in the home setting. The Speech Pathologists demonstrates the knowledge and skills necessary to provide care for the population served.

What you will do

Evaluates assigned patients through selection, administration, and interpretation of formal and informal diagnostic assessments to determine the levels of functioning and intervention needs in all areas of speech pathology.

Demonstrates knowledge of age-specific factors and performs satisfactorily related to patient’s age infant through geriatrics as noted on unit specific guidelines and competencies.

Develops and implements individualized treatment programs for the patients needing services.

Establishes and modifies intervention goals and strategies for patients/ treatment program based on on-going evaluations.

Uses selected activities and/or tasks to promote and maintain health, and to achieve treatment goals.

Provides direct therapy services to patients using various functional settings and strategies.

Plans therapy with in the patient’s individual physical, emotional and intellectual capabilities as indicated and coordinated with the rehabilitation, medical, and nursing care plans.

Requirements

Candidates must hold an active Speech-Language Pathologist license and possess CCC-SLP certification from ASHA or be eligible to complete a clinical fellowship. Experience in a hospital, SNF, or home health setting is preferred.

Required Education, License/Certification, or Work Experience

CCC-SLP - American Speech-Language-Hearing Association (ASHA) or presently active in a supervised clinical fellowship year and certification received within one year of employment Upon Hire or Speech-Language Pathologist - Licensing Board An active license in the state of practice Upon Hire

Preferred Education, License/Certification, or Work Experience: 1-3 years Speech Language Pathologist in a hospital, SNF, outpatient and/or home health setting
