Communicates with medical staff, other departments, and outside agencies while maintaining confidentiality. The position requires self-motivation, creativity, and capabilities to function in a semi-autonomous role within a fast-paced and dynamic environment.

• STANDARDS OF PERFORMANCE Assumes direct resident care responsibilities as assigned by the Charge Nurse, to include, but not be limited to AM, PM care, backrubs, assisting with ambulation, assisting with dietary needs, dressing and undressing residents, and changing bed linen.
• Carries out Special Orders or treatments as directed by the Charge Nurse, to include, but not be limited to special skincare, collecting specimens, intake and output measurements and documentation, vital signs, compresses, and enemas.
• Ensures adherence to proper infection control, OSHA and safety standards.
• Provides encouragement to residents and family members.
• Observes the resident's condition and report immediately any untoward signs, symptoms, or unusual occurrences.
• Maintains current documentation of resident status on Activity of Daily Living Sheet and gives report to Charge Nurse.
• Assists in admission, transfer, and discharge of residents.
