The Behavioral Health Care Coordinator manages a panel of patients by developing and implementing comprehensive care plans in collaboration with a multidisciplinary team. They provide advocacy, health education, and linkage to community resources to address medical, behavioral, and social determinants of health.
Candidates must hold a Bachelor's degree and a valid LPN license in the state of Ohio. Previous experience in linking consumers with community resources is preferred, along with proficiency in medical documentation and patient care procedures.

EDUCATION AND/OR EXPERIENCE: Bachelor's degree from a four-year college or university or 1-2 years of previous experience in linking consumers with various community resources preferred. Preferred educational preparation in social work or integrated behavioral healthcare. LANGUAGE SKILLS: Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization. REASONING ABILITY: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. COMPUTER SKILLS: To perform this job successfully, an individual should have the ability to gain knowledge of current practice management system, electronic medical record, Microsoft Word, text paging, Internet, and Intranet. CERTIFICATES, LICENSES, REGISTRATIONS: LPN certification. Must hold a current, valid license to practice practical nursing in the state of Ohio. OTHER SKILLS, KNOWLEDGE AND ABILITIES: Ability to speak Spanish is desirable. Knowledge of examination, diagnostic, and treatment room procedures. Knowledge of medical equipment and instruments to administer patient care. Skilled in taking accurate vital signs. Skilled in maintaining records and recording test results. Skilled with geriatric patients and patients in lower socio-economic sectors of the community.
