The LPN will provide nursing care to clients in their homes under the supervision of an R.N. Case Manager. Responsibilities include performing physician-ordered treatments, documenting care electronically, and monitoring changes in client status.
Overview We're looking for a Home Health LPN to provide practical nursing services under the supervision of a Registered Nurse.
Accepts assignments of clients from the R.N. Case ManagerProvides nursing care through the utilization of the home assessment, nursing diagnoses, and physician-approved care plansa. Performs treatment(s) ordered by the physician and delegated by the R.N. Case Manager. b. Observes signs and symptoms and reports them to the R.N. Case Manager. c. Notes changes in physical or emotional status of clients. d. Promotes client and caregiver independence in self-care Utilizes electronic documentation of client care; complies with Agency policies regarding timeliness in submission of documentation and synchronization of dataFollows Agency policies regarding universal precautions, and safety guidelinesFulfills other assignments as requested by the R.N. Case Manager, VP of Clinical Services, or Director of Professional Services.

Candidates must hold a current Licensed Practical Nurse license in Virginia and possess at least one year of nursing experience. A current CPR certification and strong communication and organizational skills are also required.
