This role focuses on housing stability, daily functioning, and overall wellness through regular engagement, individualized planning, and coordination of services. Working within a residential setting, the Case Manager builds relationships with Veterans, identifies barriers to stability, and connects individuals to appropriate internal teams and external service partners. This role supports Veterans participating in structured residential housing programs focused on stabilization and transition to permanent housing. The Residential Case Manager manages an assigned caseload, providing consistent support from intake through transition planning and discharge, while collaborating with clinicians and program staff to ensure coordinated, effective care. The Residential Case Manager plays a key role in maintaining a safe, supportive environment by recognizing potential risk factors, responding to emerging concerns, and coordinating with clinical staff or emergency services when appropriate. This role requires balancing direct client engagement with timely, accurate documentation to meet program and compliance requirements. This is a fully on-site role based in a residential setting, supporting daily operations and Veteran needs. Key Responsibilities Veteran Support and Housing Stability Provide direct case management support to Veterans in residential housing, focusing on daily stability and progress toward individualized goals. Build professional relationships with Veterans through regular engagement to support trust, accountability, and participation in the program. Help Veterans identify barriers to stability, including housing, employment, behavioral health, and daily living needs. Work with Veterans to develop and review individualized plans that support stability, independence, and long-term progress. Care Coordination and Service Support Coordinate with clinicians and program staff to align services and ensure Veterans receive the support they need. Connect Veterans to internal services and external resources based on their needs. Make referrals and follow up to help Veterans access behavioral health care, housing resources, and other services. Monitor Veteran progress through regular check-ins and communication with the care team. Residential Oversight and Support Maintain a consistent presence in the residential setting to support a safe and structured environment. Recognize changes in Veteran behavior or stability and respond by involving the appropriate team members or escalating concerns when needed. Reinforce program expectations, structure, and community standards as part of daily interactions. Documentation and Compliance Maintain timely, accurate, and confidential documentation, including case notes, service plans, referrals, and progress updates. Complete documentation in line with program requirements and audit standards. Track Veteran progress, services provided, and outcomes to support reporting and program needs. QualificationsRequirements Associate or bachelor’s degree in social work, human services, or a related field preferred; equivalent experience will be considered. 1-3 years of experience in case management or a related direct-service role, preferably within Veteran services or the human services field. Strong organizational, communication, and problem-solving skills. Ability to build rapport and effectively engage with Veterans from diverse backgrounds. Ability to work independently and collaboratively with at-risk populations. Proficiency in data management systems, such as HMIS or similar platforms. Reliable transportation and driver’s license required. Ability to manage a documentation-heavy workload in a fast-paced, compliance-driven environment. Must pass a background check.

Preferred Qualifications

Military or Veteran experience is not required but is strongly valued. Experience working in residential, transitional housing, or shelter-based programs. Knowledge of homelessness prevention, rapid re-housing, and transitional support services. Ability to remain flexible and adapt to changing environments. Schedule Monday – Friday; 8:00 a.m. to 4:00 p.m. Occasional evenings or weekend support may be required based on program needs. Compensation The salary range for this position is $55,000 - $58,000 annually. The final offer will be based on experience, qualifications, and alignment with program funding. Requirements
