Assumes responsibility for daily coordination of the resident's care, including comprehensive assessments, care conferences, and written plans of care. The RN-MDS Coordinator oversees, coordinates, & assumes responsibility for resident care which includes but is not limited to admission, discharge, and transitions of care. The MDS Coordinator assures that quarterly, annual, & significant change observations & assessments are completed in a timely fashion for assigned group of Residents. Member acts as a resident advocate, provides leadership, organization, and direction to the nursing department.
What you will do
Audits resident charts, care plans, and charting to ensure documentation supports the care provided and reimbursement level.
Completes and transmits MDS assessment and reports in accordance with regulatory requirements and facility policies and procedures.
Identifies and communicates need for significant change in status assessments.
Ensures compliance with guidelines for participation in Medicare, Medicaid and other benefit programs.
Maintains a viable working relationship with all disciplines involved in the planning of resident care.
Completes Discharge Assessment and entry/re-entry tracking forms per Federal regulations.
Schedules MDS assessments per guidelines, as assigned and maintains a monthly calendar.
Notifies members of the IDT of changes or additions to monthly Care Plan Meeting schedule, for new admission, change of condition or interim meetings.
Electronically submits MDS information. Reviews, revises, corrects, and resubmits as appropriate;. Contributes to the Interdisciplinary care plan by communicating any resident issues that are pertinent to the plan of care.
Participates in facility QA initiatives.
