Avail Health – Contract PRN Mobile Nurse Practitioner - West Virginia, South Region
Avail Health is a Nurse Practitioner–founded organization with a mission to remove barriers: to care, to access, and to meaningful work. We combine thoughtful technology, strong operational infrastructure, and clinically driven programs to enable high-quality, consistent care delivery at scale, allowing our teams to focus on delivering impactful, outcomes-oriented care where patients live.
Avail Health partners with Medicare Advantage health plans to support Medicare-eligible individuals, including those in long-term care settings, through clinically driven programs focused on proactive care, accurate clinical documentation, and improved health outcomes. Our work is grounded in empowering clinicians, reducing administrative burden, and enabling consistent, high-integrity care delivery that enhances both patient experience and care team effectiveness.
This is a 1099 contract position.
This role supports Avail Health’s partnership with an Institutional Special Needs Plan (I-SNP) Medicare Advantage health plan. As a mobile Nurse Practitioner, you will conduct in-person Chronic Disease Assessments using historical data provided by the health plan and in-person evaluation to support accurate clinical documentation of chronic conditions for health plan members residing in long-term care facilities. You will also complete Health Risk Assessments with the member and their authorized representative (if applicable) to evaluate health risks and develop Individualized Care Plan recommendations (problem, goal, interventions). These assessments and care plan recommendations are used by the health plan to support coordination with the interdisciplinary care team responsible for the member’s ongoing care.
This is a highly collaborative, patient-facing role that requires strong clinical judgment, effective communication, and close partnership with Avail Health operations to support scheduling, documentation, and overall program execution. Comfort working independently in the field is essential, along with flexibility and a willingness to contribute ideas and feedback to support continuous improvement in a dynamic, growth-oriented environment.
Engagement will begin upon execution of the agreement and continue through June 30, 2026. Thereafter, there may be an opportunity to extend the engagement in successive 3-month increments aligned to calendar quarters, based on program needs and performance.
This is a PRN (as-needed) opportunity designed to offer flexibility based on your availability and local patient needs. Service volume may vary depending on patient census and regional demand, with anticipated utilization averaging approximately 2 days per week within your assigned geographic area.
Contractors have the flexibility to designate their available weekdays (Monday–Friday, 8:00 a.m.–5:00 p.m.) for completing on-site patient visits. Availability is submitted in advance to support monthly scheduling, with visits aligned to clinician availability and program visit targets.
