The LCSW is responsible for assessing patient needs, developing treatment plans, and coordinating clinical management from admission to discharge. They provide individual, family, and group therapy while ensuring compliance with hospital and regulatory documentation standards.
Candidates must hold a Master of Science in Social Work and a current LCSW license for the State of Illinois. The role requires 3-5 years of experience, proficiency with EHR systems, and current BLS certification.
The LCSW is responsible for assessing and coordinating the clinical management of patients in specific case groups for an episode of care. The LCSW is responsible for developing and meeting desired patient care outcomes for his/her caseload based on assessment of patient care needs and as ordered by the physician, within an appropriate length of stay and appropriate use of resources. The LCSW responds to complex patient care needs from admission to discharge, works productively and cooperatively in collaboration with the physician and other members of the interdisciplinary team.
