The Intake Coordinator performs administrative and office support for multiple departments and assists the Admissions Coordinator with putting together new patient charts. They facilitate communication between patients, families, and other team members while maintaining confidentiality of records.
Basic computer skills are mandatory, and experience in home health is preferred. Spanish speaking is a plus, and the company is willing to train motivated candidates.

Facilitates communication between patients / families and other team members.
Communicates with staff and outside agencies as directed.
Maintains confidentiality of records ad is cognizant of patient and employee privacy rights.
Monday - Friday 9am - 530pm
Full Time position with PTO / Sick Time / Holidays / Health Insurance / 401K
Previous Home Health Experience a plus but will train motivated candidate.
