The Intensive Case Manager will assist clients with housing applications, move-in processes, and long-term stability support. They are responsible for conducting regular home visits, maintaining accurate documentation in HMIS, and collaborating with community service providers.
A Bachelor’s degree in a human service field such as social work is required, though equivalent experience will be considered. Candidates must have a valid Texas driver’s license, reliable transportation, and auto liability insurance.
Recovery Resource Council is looking for a conscientious, detail-oriented, and compassionate individual. This person must be capable of successfully managing multiple tasks and responsibilities while working with previously chronically homeless individuals who have received a housing voucher.
To perform this job successfully, an individual must have a Bachelor’s degree in a human service field such as social work; equivalent experience will be considered. Experience working with people experiencing homelessness is preferred. Must have reliable transportation, a current Texas driver’s license and evidence of auto liability insurance.
The Intensive Case Manager will become knowledgeable of eligibility criteria of clients to become participants in the Shelter 4Hope housing case management program. They will assist clients with the application process and move-in with the housing authority. This includes help with documents, facilitating their attendance at the housing authority briefing meeting, searching for a unit, accessing resources to set up a household, and moving in. Once the client moves in, the Intensive Case Manager will provide on-going, long-term case management to support the client’s housing stability. This includes a minimum of 2 home visits per month, along with any additional supports or contact with the client. Case management includes setting mutually agreed upon goals that increase the success of maintaining housing. The Intensive Case Manager assists with any barriers to these goals, finds supportive resources, and provides education and guidance depending on the issue at hand. The Intensive Case Manager must also enter data into HMIS on a timely basis, including assessments and case notes, providing complete documentation of all services provided, including eligibility verification, consents, client contacts, progress notes, and service plan updates. They will also provide information as necessary for collaboration with funding sources and for evaluation purposes. The Intensive Case Manager will also participate in community meetings, collaborating with and supporting the efforts of the wider homeless service provider network. The Intensive Case Manager will also establish and promote good working relationships with apartment complexes for the benefit of the client and the property. The Intensive Case Manager must be able to work with and interact with clients and other stakeholders from a variety of ethnic, cultural, and economic backgrounds, adhering to the agency guarantee to provide everyone with “unconditional acceptance, respect, and positive regard."

This is a hybrid position, requiring both remote and in-person work, involving sitting and standing for various lengths of time and frequently driving various distances.
