The provider will perform physical exams, diagnose and treat acute and chronic conditions, and develop individualized care plans for residents in long-term care facilities. They will also collaborate with interdisciplinary teams and educate patients and families on treatment goals.
Candidates must hold an active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license in New York. A minimum of 6 months of experience in long-term care or a skilled nursing facility is required.

Infinite Medical Group is currently hiring a Primary Care Nurse Practitioner/Physician’s Assistant (NP/PA) to provide on-site care to residents in long-term care settings.
