Service Coordinators provide flexible, community-based support to individuals experiencing mental health challenges and homelessness to promote stability and recovery. They act as integral members of the ACT team, managing caseloads and assisting clients with life skills, housing navigation, and benefit advocacy.
Candidates must have a strong understanding of mental health and substance use issues, along with excellent communication and documentation skills. A valid driver's license, reliable personal vehicle, and the ability to lift 50 pounds are required.

Service Coordinators are members of the interdisciplinary team who provide flexible, compassionate, community-based services that are designed to promote the stability, recovery, and community reintegration of individuals who experience mental health challenges, histories
of homelessness, and may also have challenges around substance use. Service Coordinators function as integral members of Pathways’ Assertive Community Treatment (ACT) team which utilizes an interdisciplinary staff to serve a combined caseload of clients and communicates with community partners.

Schedule: Full time (35-40 hours per week)
