  • You will: Deliver direct and indirect patient care using the nursing process—Assessment, Planning, Intervention, and Evaluation Collaborate closely with patients, families, and our interdisciplinary care team to create personalized care plans Monitor and communicate changes in patient conditions to physicians, supervisors, and colleagues Play a key role in the discharge planning process to support safe, successful transitions Your skills, compassion, and critical thinking will help patients achieve greater independence, strength, and quality of life.

Minimum six months’ Medical/Surgical experience in an acute care setting preferred. Excellent oral and written communication and interpersonal skills. Requirements

Candidates must have graduated from an accredited nursing program and hold a current state licensure as a Licensed Practical Nurse. Additionally, a CPR/BCLS certification is required, and six months of acute care experience is preferred.
