Coordinate long-term care needs for patients by assessing, planning, and implementing individualized care strategies. Act as a single point of contact to manage care plans and advocate for patients to ensure they receive appropriate health and social services.
Requires a current unrestricted RN license in Texas and at least 4 years of community health experience. Candidates must have 2+ years of experience with maternal and infant populations and reside in Texas.
Current RN unrestricted license in the State of TX 4+ years of experience working within the community health setting in a health care role 2+ years or equivalent of experience working with Maternal and Infant population such as Mother/Baby, Labor and Delivery, or Neonatal Intensive care Unit Must reside in the state of Texas
5+ years of experience documenting in an electronic documentation system 5+ years of experience working with MS Word, Excel and Outlook Case Management experience including but not limited to working with Special Health Care Needs Populations, Complex medical conditions Pediatrics Certified Case Manager (CCM) Experience in discharge planning Experience in utilization review, concurrent review or risk management Experience with arranging community resources Background in managing populations with complex medical or behavioral needs *All Telecommuters will be required to adhere to UnitedHealth Group’s Telecommuter Policy.
