The Physical Therapist will assess, evaluate, and deliver therapy services to patients in their homes to improve function and quality of life. They are responsible for developing care plans, educating caregivers, and maintaining accurate clinical documentation.
Candidates must hold a bachelor's or master's degree in physical therapy and possess a valid state license. Two years of relevant experience is preferred, along with current CPR certification and a valid driver's license.
As a Home Health Physical Therapist, you are a critical part of the care team - helping patients safely recover, regain mobility, and maintain independence where they feel most comfortable: at home.
This is a highly autonomous, relationship-driven role where you can truly see the impact of your work every day.
